Clinical Review of Early Gastric Cancer




Abstract
Stomach cancer is one of the most prevalent malignancy in Korea, but the prognosis of this cancer is very poor despite of improved surgical technique and advanced diagnostic method.
However, the prognosis of early gastric cancer is excellent. So, we emphasize on early diagnosis and treatment of gastric cancer.
This study analyzed 32 cases of early gastric cancer treated at the Department of General Surgery,
Chung Ang University Hospital during the past 6 years from January 1984 to December 1989.
1) The proportion of EGC among total operated stomach cancer was 13.4%. 2) Male to female ratio was 2.6: 1 and peak age incidence was in 6th decade.
3) On macroscopic classification, type IIc was most frequently encountered (65.6%).
4) In 17 case (53.1%), the cancer were limited to mucosa and 15 cases (46.9%) invaded to submucosa.
5) Most common affected site was antrum (59.4%), followed by body (31.3%) and pylorus (9.3%).
6) The size of the tumor was varied between 0.5 x 0.5 cm and 4.0 X 6.5 cm and its average size was 2.3 cm.
7) Histopathologic classifications were revealed well differentiated adenocarcinoma; 12 cases (21. 9%), moderately differentiated adenocarcinoma; 3 cases (9.4%), poorly differentiated adenocarcinoma; 9 cases (28.1%), signet ring type; 7 cases (21.9%), and mucinous type; 1 cases (3.1%).
8) Lymph node metastasis was present in 2 cases (6.3%).
9) Radical total gastrectomy was performed in 6 cases and the remainder underwent Radical subtotal gastrectomy.
10) Post operative complications were occurred in 3 cases, which included toxic hepatitis in 2 cases and intestinal obstruction in 1 case.
11) Among 32 cases, only 26 cases could be followed up, 19 cases by visiting personally and 7 cases answered to our request form, and they are now all alive without any recurrence.
